Medical effect: Topina tablets are effective for the treatment of partial seizures in combination with other antiepileptic medicines
Contraindications and precautions: do not use for the following categories of patients:
- pregnant and breastfeeding women,
- patients with glaucoma,
- patients with renal or hepatic disorder,
- patients with depression.
Patients prone to the calculi should drink an adequate amount of water and have a constant monitoring of the calculi state by the consulting doctor.
If an allergic reaction occurs, stop taking the medicine and consult with your doctor. If you’re taking any other medication, please consult with your doctor before use.
As this medicine may cause sleepiness, avoid driving a car or operating dangerous mechanisms while taking it. The medicine may decrease sweating and increase body temperature, especially during hot season. Avoid staying at a high temperature and drink water beforehand to relieve symptoms.
The medicine may cause decreased body weight. If you noticed any signs of unexpected weight loss, consult with your doctor.
Avoid taking foods containing Hypericum perforatum (St. John's wort) while taking this medicine.
Dosage and administration of Topina tablets 100 mg from Japan for epilepsy (topiramate)
For adults: start with taking 50 mg of the active ingredient (0.5 tablet) 1-2 times a day and continue to take it so for more than a week. Then the dose should be increased gradually up to 100-200 mg 2 times a day as a maintenance dose. The dosage may be adjusted by the consulting doctor according to the symptoms. However, the maximum daily dose of 600 mg should not be exceeded.
For children aged 2 years and older: start with taking 1 mg of the active ingredient per kilogram of the body weight daily and continue to take it so for more than 2 weeks. Then the dose should be increased to 2 mg per kilogram of the body weight daily and be continued for more than 2 weeks. Gradually increasing the dose by 2 mg per kilogram of the body weight at a step, the dose should be increased up to 6 mg per kilogram of the body weight as a maintenance dose. The dosage may be adjusted by the consulting doctor according to the symptoms. However, the maximum daily dose should not exceeded 9 mg per kilogram of the body weight or 6 tablets (600 mg), whichever is less. In any case, the medicine is taken daily in 2 divided doses.
The medicine is supposed to be used in combination with other antiepileptic medicines. Please note that one Topina tablet contains 100 mg of the active ingredient.
How effective are Japanese Topina tablets 100 mg for epilepsy (topiramate)?
Topina tablets contain topiramate, a sulfamate modified fructose diacetonide. It is sold upon such brand names as Topamax, Trokendi XR, Qudexy XR and others. Topina tablets calm excessive nerve excitement in the brain and suppress partial epileptic seizures. Studies also showed, that topiramate is effective for inhibiting maximal electroshock and pentylenetetrazol-induced seizures as well as secondarily generalized tonic-clonic seizures in the kindling model (A. P. Kudin, G. Debska-Vielhaberet al. "The mechanism of neuroprotection by topiramate in an animal model of epilepsy". Epilepsia. 2004, 45 (12): 1478–87).

Topina tablets can be used for the treatment of epilepsy in both adults and children. Unlike other antiepileptic drugs, like levetiracetam, carbamazepine or lamotrigine, topiramate does not cause apoptosis in young organisms. Also, topiramate is currently studied as an effective medicine for various diseases, including medication overuse headache, bipolar disorder, alcoholism and others. Topina tablets also can be prescribed for the prevention of migraines, the dosage is indicated by the consulting doctor.
