What are Parmodia XR Tablets 0.2 mg?

Parmodia XR Tablets 0.2 mg are prescription extended-release tablets for hyperlipidemia, including familial forms. Each pale yellow, round tablet contains 0.2 mg of pemafibrate. The XR tablet is a multiple-unit formulation, which means a tablet containing granules that release the medicine slowly. This allows one dose a day. Pemafibrate was first available as an immediate-release tablet taken twice daily, and the XR tablet was developed for once-daily use and approved in Japan in June 2023. XR tablets and the 0.1 mg immediate-release tablets are separate products with separate dosing.

What is hyperlipidemia?

Hyperlipidemia means too much fat in the blood, most often high triglycerides or high LDL cholesterol. High triglycerides are linked to arteriosclerosis, the hardening and narrowing of arteries. Pemafibrate is used mainly when triglycerides are high. In patients whose LDL cholesterol is high and triglycerides normal, it is not a first-choice medicine. It is considered when statins cannot be used because of past side effects or other reasons.

How does pemafibrate work?

Pemafibrate is a selective PPARα modulator. PPARα is a receptor in liver cells that regulates the genes controlling how fat is made, released and cleared. Pemafibrate binds to PPARα far more strongly than to related receptors (PPARγ and PPARδ), so it works selectively.

In laboratory studies in rats, it reduced triglyceride production in the liver and slowed their release into the blood. It also raised the activity of lipoprotein lipase, the enzyme that breaks down triglycerides in the blood, and it lowered the level of two proteins that inhibit that enzyme, ApoC-III and ANGPTL3. Plasma triglyceride clearance increased. In mice, it raised HDL cholesterol and reduced fatty deposits in the aorta. It also lowered cholesterol absorption in the intestine, and absorption markers fell in patients with fatty liver.

The extended-release design changes how the medicine appears in the blood. In patients taking 0.4 mg a day, the peak level occurred around 3 to 8 hours after the dose, later than with immediate-release tablets, depending on meal timing.

What clinical studies show

The main trial of the XR tablet was reported in Arai et al., “Efficacy and Safety of Pemafibrate Extended-Release Tablet: a Phase 3, Multicenter, Randomized, Double-Blind, Active-Controlled, Parallel-Group Comparison Trial,” Journal of Atherosclerosis and Thrombosis, 2024. It enrolled 356 patients with fasting triglycerides of 200 mg/dL or more. For 12 weeks they took XR 0.2 mg or 0.4 mg once daily, or immediate-release 0.2 mg a day in two doses. Triglycerides fell by 43.8% with XR 0.2 mg, 48.0% with XR 0.4 mg and 48.0% with the immediate-release tablet. Both XR doses were non-inferior to the twice-daily tablet.

Long-term data come from Arai et al., “Long-Term Effects of Extended-Release Pemafibrate Tablets on Dyslipidemia and Safety in Triglyceridemic Patients: A Phase 3, Multicenter, Randomized, Open-Label, Parallel-Group Study,” Journal of Atherosclerosis and Thrombosis, 2025. It enrolled 121 patients who took XR 0.2 mg a day, in the morning or in the evening, for 52 weeks. Triglycerides fell by 44.8% in the morning group and 46.6% in the evening group, and the difference between timings was not significant. The dose was raised in 82 patients with an initially inadequate response, and 57.3% of them reached a fasting triglyceride level below 150 mg/dL.

A study of how the tablet behaves in the body was reported in Yamashita et al., “Clinical Pharmacology of Pemafibrate Extended-release Formulation in Patients with Hypertriglyceridemia—A Phase 2, Multicenter, Active-controlled, Randomized, Single-blind, Crossover study,” Journal of Atherosclerosis and Thrombosis, 2025. In 60 patients treated, the XR tablet reached its peak later and had a longer half-life than the immediate-release tablet, and the triglyceride-lowering effect tended to be stronger when taken after a meal than on an empty stomach. No safety concerns arose at doses up to 0.8 mg a day.

Dosage and administration: The usual adult dose is one XR tablet of 0.2 mg once a day. Depending on how high triglycerides or LDL cholesterol are, your doctor may raise it to 0.4 mg once a day. If your kidney function is low (eGFR below 30 mL/min/1.73 m²), your doctor decides carefully whether the medicine is suitable, and if used, the dose is 0.2 mg once a day. XR tablets are extended-release, so swallow them whole. Do not crush, grind or chew them. In studies, tablets were taken in the morning or in the evening, and before or after meals made little difference to blood levels. Diet and exercise remain the basis of treatment. Blood lipids are checked regularly, and your doctor reviews whether to continue. The dosage can be adjusted by your doctor.

 Do not use if you have had a hypersensitivity reaction to any ingredient, severe liver disorder, cirrhosis of Child-Pugh class B or C, biliary obstruction, gallstones, or if you are pregnant or may be pregnant. Do not use while taking cyclosporine or rifampicin. Tell your doctor and pharmacist about all medicines you take. Clopidogrel, clarithromycin, ritonavir and other HIV protease inhibitors, and fluconazole raise pemafibrate levels. Bile acid binders (cholestyramine, colestimide) lower them, so take them as far apart as possible. Strong CYP3A inducers (carbamazepine, phenobarbital, phenytoin, St. John’s wort) may weaken the effect. Breastfeeding is best avoided. No clinical trials have been done in children. In elderly patients, treatment proceeds with careful observation.
